How to Build an Audit Evidence Folder
Structure evidence so it is current, relevant and retrievable.
Start with the system
Start with an outcome-based index. For each applicable outcome, identify a controlled document and a focused set of operational examples.
Put it into practice
Minimise personal information, apply role-based access and prepare appropriately redacted exports. Do not create or backdate evidence for an audit.
Review and improve
Assign an owner to check links, versions and access. Record gaps in corrective action and follow them through.
